On March 12, 2026, DeFi experienced one of its biggest 'oops' moments in history. A trader (probably an experienced whale) attempted to exchange about $50.43 million in aEthUSDT (the deposited version of USDT on Aave) for aEthAAVE tokens via the official Aave interface and the CoW Protocol router.

Result? He received only *324 to 327 $AAVE … barely $36,000 at market price (around 111 $USDT per token at that time). A net loss of nearly $50 million* in just a few seconds.

No hack, no exploit. Just a monstrous slippage that the user voluntarily confirmed despite the ultra-clear warnings from the interface ('extraordinary slippage' + mandatory checkbox). The founder of Aave, Stani Kulechov, even offered to reimburse the transaction fees (around $600,000) as a goodwill gesture. But the $50M? Unrecoverable. It’s pure blockchain: once confirmed, it’s engraved for eternity.
