I will be honest: What made this feel real to me was a “near miss” that never became a headline. A robot hesitated in the wrong place. Nothing happened. No damage, no injury. But the incident report still got filed, and that’s when the whole thing turned into a paperwork storm. Not because anyone was panicking — because everyone knew what comes next: auditors, insurers, possibly regulators. And the only question that mattered was boring and brutal: who approved the behavior that led to this?

That’s the practical issue when autonomous robots and AI agents operate across organizations. Decisions are distributed. The model comes from one party, the deployment pipeline from another, the on-site overrides from a third, and the “rules” from a safety team that may not even touch the code. The robot’s behavior is the sum of all of it. But responsibility still needs a name, a signature, a trail. Law doesn’t accept “it emerged from the system” as an answer.

Most current solutions feel flimsy in practice. Internal logs don’t align across company boundaries. Vendor dashboards don’t capture local changes. Tickets can be incomplete, delayed, or written to justify outcomes. And people behave predictably under risk: they document selectively, they avoid admitting ownership, and they optimize for plausible deniability once money is on the line.

The first users are regulated operators who already pay for audits: hospitals, logistics fleets, public deployments, insurers. It might work if it’s cheaper than today’s evidence hunt. It fails if it’s optional, or if participants don’t accept the shared record as real when it hurts.
